Transition Timeline

Transition planning encompasses a wide variety of elements including education, employment and independent living. Knowing when to start planning and different milestones is key to ensuring all aspects of transition planning are covered. The Transition Timeline identifies the activities and actions to consider for the core transition years ages fourteen to twenty.

  • Consider involving the student more fully in the IEP process and the IEP meeting.

  • Discuss graduation options (Certificate of Completion/Diploma)

We are all unique in our own way.

Stephen has technology to thank for being in college, and he is giving back. His self-designed independent major at Keene State College—Disability Studies—will allow him to work in the assistive technology field, offering training and technical support for other blind and visually-impaired computer users. Currently, Stephen advises fellow students in the field of technology, and his enthusiasm is noticeably contagious.
